After this date, the clinic will no longer offer ongoing audiology services.
Patients currently in active treatment or within a hearing aid trial period will continue to receive appropriate care through completion of their treatment plan.
Patients who require continued audiologic care beyond this date may establish care with another local provider.
Patient records will be securely maintained in accordance with state and federal regulations. Requests for records may be submitted in writing to the contact information listed below.
This notice is provided to ensure continuity of care and adequate time for patients to make alternative arrangements.
